We usually see patients by appointment only, however should you develop a serious problem or emergency with your eyes, please call our office IMMEDIATELY. We will make sure you are seen without delay. After office hours our telephone exchange will contact the physician on call for emergencies. Non-emergency calls will be returned the next business day.

 
Phone Calls ~
Your phone calls are important to us. Every question you have whether medical or administrative deserves an answer. To ensure that the information you need, our doctors work closely with our technicians and staff who are trained to answer most questions. Routine questions about your eyes or inquires about medication and prescription renewals should be made during hours when your medical records are available. If you need to speak to one of the physicians by telephone, the staff will arrange a time.

First Visit ~ Your first visit to Eye Life Institute will be extensive - please allow 1 ½ hours. We will ask you to fill out our patient information sheet. Your general health will be reviewed and we will perform a comprehensive eye exam, which includes dilation. Dilation can cause temporary vision blurring and light sensitivity. Please bring a pair of sunglasses to wear after your exam.

Payment is due at time of service. We bill most insurances.
 

 
What to bring to your appointment

    Any eye drops you may be using, prescription or over-the-counter
    A list of all present medications
    A pair of sunglasses to protect your eyes after dilation
    Your regular prescription glasses - we will clean and adjust them
    Your insurance cards, both primary and secondary - copies will be made for our records
    HMO authorization or insurance forms if required
    Payment for deductible/copay and refraction
    Ride home for your safety - dilation can cause temporary vision blurring and light sensitivity

Your time is valuable to us. Occasionally, an emergency may mean our office schedule runs a bit late. If this happens, we'll make every effort to contact you prior to your appointment so you need not wait as long in our office.

During your visit stop by our optical department. We will. . .
  - clean and adjust your current eyewear
  - help fill your new prescription
  - update you on new technology and fashion in the optical field
  - encourage you to select stylish sunglasses to protect your eyes.

Important information ~ Part of your comprehensive eye exam is the refraction. It is a separately billed service for your eyeglass prescription. This prescription has an expiration date of 1 - 2 years depending on the health of your eyes. A refraction also assists the Doctor in determining the cause of vision need. Our Doctors recommend this procedure to be completed during each exam so that you will have a current prescription in the case of loss or breakage, and so the Doctor can gather important information concerning your vision. Many insurance companies do not cover this cost so please expect to pay for this procedure at time of service.
